I know it requires an xm antenna. Will it require a new tuner ? Will it need programming ?

Of course......you will have to pay for an XM subscription and you are changing the head to get a new tuner.
If your present head has an AUX input, it might be a LOT easier to get an "outboard" XM tuner.

I have other vehicles with xm and pay the fee's. However, if I install this radio head from a prius that has xm capability of the same year will I need the tuner as well ? If so, will it work without needing the dealer to do anything ? radio head

NO. The fact that the head has XM capability, means that it already HAS the necessary tuner.
Or it should mean that.

You will have to contact XM with it's serial number to pay the fee and get it activated though.
